This guide outlines Transforma's Monitoring and Evaluation (M&E) policy, detailing the mechanisms used to track project progress, measure impact through SMART indicators and theories of change, and ensure institutional accountability across all organizational processes.

• Transforma employs a two-part approach to measuring intervention impact: periodic monitoring and auditing to identify gaps and make adjustments during execution, followed by objective evaluations of results after the intervention is completed.
• To measure impact, every Transforma project is required to have an explicit theory of change and a set of "SMART" impact indicators that can be tracked at the project's end and in the medium term, including any indicators required by donors or contractors.
• Monitoring and auditing activities are carried out by project teams under the supervision of the Project Director using periodically updated databases, forms, and surveys to evaluate the fulfillment of initial objectives.
• The organization ensures policy adherence through induction training for new members and annual training sessions or materials for Project Directors. The policy is subject to updates based on legal changes or organizational shifts.
